Apply for your medical marijuana card
To apply for a Kentucky medical marijuana card, patients must complete the application available on the Kentucky Medical Cannabis Program’s website, which will be available by January 1, 2025. The application requires specific personal information, including the applicant's full name, address, phone number, email, date of birth, Social Security number, and driver's license number.
Once the form is completed, applicants must pay a $25 application fee. A notarized signature page is also required, affirming that the applicant will not divert medical cannabis to unauthorized individuals. By fulfilling these steps, qualified patients can gain legal access to medical cannabis in Kentucky.
