What do I need to qualify for the Kentucky medical marijuana program?

  • You must be a Kentucky resident in order to get your Kentucky cannabis card. If the patient is a nonemancipated youth under the age of eighteen (18), the parent or legal guardian must provide written consent at time of appointment, and serve as their caregiver.

  • You must suffer from at least one of the following conditions:

    • Any type or form of cancer, regardless of stage;

    • Chronic or debilitating pain

    • Epilepsy or any other seizure disorder;

    • Multiple sclerosis, muscle spasms, or spasticity;

    • Chronic nausea or cyclical vomiting syndrome that has been proven resistant to other conventional medical treatments;

    • Post-traumatic stress disorder

  • Patients must provide some form of medical records to help the marijuana doctor or nurse practitioner perform the medical marijuana evaluation. You should try to provide at least one of the following:

    • Records or notes from a doctor’s visit

    • Discharge summaries from a hospital visit

    • Referrals or notes from a chiropractor or other medical professional

    • Current prescription medication, or your prescription history printed from your local pharmacy

    • Proof of prior medical marijuana status in another state

  • At your appointment, our medical marijuanas doctor (or nurse practitioner) will evaluate you based on your current health condition(s), medical history, and any documentation you are able to provide to determine if you qualify for medical cannabis use. Afterwards, you'll receive your written certification form. In January, you will be eligible to apply and receive your Kentucky medical marijuana card!
